Outreach Service

SignHealth provides outreach services to support Deaf people with more independent living, such as help with difficult tasks and activities. 

Our Outreach Teams help vulnerable Deaf people (including Deaf people with mental health illnesses) to lead independent lives, by giving support in difficult tasks at home and elsewhere. We develop a personalised plan for each client and work closely with other agencies. 

All our outreach workers are Deaf or fluent in BSL, and all have experience working with Deaf people who have mental health problems. This means it is easier to form a relationship with clients, and identify what support is needed. 

If a Deaf person is receiving care in a hearing establishment, we can go to them to provide our service, and to deliver Deaf awareness training to the hearing staff. 

How We Help

Our work varies from client to client. We carefully match our Outreach workers to meet the client’s specific needs.

We offer a wide variety of support, including:

  • Learning skills for daily living
  • Budgeting
  • Applying for jobs 
  • Getting out of the house
  • Taking a course
  • Accessing other services or resources
  • Joining a club or social group

For example, if you need help with your grocery shopping, we can support you in deciding what you want to buy and how to plan a week of meals. We offer support in planning a trip to the shops, and tips for getting the shopping done. It means our clients gain confidence to do things by themselves.

Who Uses The Outreach Service?

We support a variety of Deaf clients, but most have a mental health problem impacting their independence and life skills. 

Social services pay for the Outreach Service. 

What Makes Our Service Different?

All of our outreach workers are either Deaf or fluent BSL signers. This means they are able to form a proper relationship with their clients, and understand what support is needed. Our clients prefer having someone who can sign as it is far easier and more personal than communicating through an interpreter. 

Our Outreach Workers all have experience with Deaf people who have mental health problems and can spot whether someone’s health is getting worse and take action to help.

Referrals

We take referrals from GPs, local authorities and social workers.

Please download and complete the referral form and then send it to your nearest outreach team using the contact details listed below.

After the referral process is accepted and complete, we organise a face-to-face assessment with the client and together devise a personal plan for the kind of support they need.
